(Palm Beach Gardens, FL) — Lisa Izquierdo, RN, NHA, has recently been named the new Director of Continuing Care at Chatsworth at PGA National, the continuing care neighborhood associated with Devonshire at PGA National, an Erickson Living community in Palm Beach Gardens, FL. Izquierdo brings to Chatsworth more than 17 years of leadership expertise in the healthcare industry. She officially started work in her new position on January 11.

Prior to joining Chatsworth, Izquierdo was most recently the Interim Executive Director/ Executive Director at The Village on High Ridge in West Palm Beach. Additionally, she has served as the Executive Director for Consulate Healthcare Management in West Palm Beach and Pompano Beach. Izquierdo’s other positions have included the Director of Nursing Services at John Knox Village Health Center in Pompano Beach, as well as the Nursing Home Administrator and Director of Nursing Services for HCR Manor Care in Boca Raton and Lauderhill.
Izquierdo received a Bachelor of Health Services Administration degree from Florida Atlantic University and a Bachelor of Science degree in Nursing from Florida International University. She is a registered nurse and a licensed Nursing Home Administrator in the state of Florida.
In 2015, Chatsworth at PGA National receive a five-star overall rating by The Centers for Medicare and Medicaid Services; the rating is an assessment based on a variety of categories including health inspections and quality of care, among others.
